A noble profession should require its students and graduates
to swear an oath revering patients\u2019 rights and autonomy.<\/mark><\/strong><\/em><\/p>\n\n\n\n

In the mid\u2010\u200b20th century, medical schools began administering modernized versions of the oath<\/strong>, more applicable to modern times and sensibilities<\/strong>. In the last 20 or so years, many medical schools have created unique versions of the oath, often allowing students to compose them. These newer versions stray far from the oaths<\/strong> that older\u2010\u200bgeneration doctors like me recited. Some have shifted the emphasis from patient care to social justice, generating a firestorm of controversy.<\/strong><\/em><\/p>\n\n\n\n

Yet all these oaths\u2014traditional or modern\u2014are self\u2010\u200bindulgent.<\/mark><\/strong><\/em><\/p>\n\n\n\n

They\u00a0focus<\/strong>\u00a0primarily on\u00a0how physicians should comport themselves<\/strong>, relate to professional colleagues, and view the medical profession\u2019s role. But they\u00a0also regard patients similarly to how parents regard children.<\/strong><\/em><\/p>\n\n\n

The original oath states,<\/strong> \u201cI will prescribe for the good of my patients\u2026 and never do harm<\/strong> to anyone\u2026 nor give advice which may cause his death.\u201d But it also pledges to impart to \u201cthe sons of the master who taught me and to the disciples who have enrolled themselves and agreed to the rules of the profession, but to these alone, the precepts and the instruction\u201d<\/strong> (emphasis added) anticipating the protectionism of cartelized modern medicine<\/strong>.<\/em><\/p>\n\n\n\n

The\u00a0Declaration of Geneva<\/a>,<\/strong>\u00a0composed by the World Medical Association, states \u201cthe health of my patient will be my first consideration,\u201d while \u201cmaintaining the honor and noble traditions of the medical profession,\u201d but makes\u00a0no mention of informed consent or respecting patients\u2019 choices.<\/strong><\/em><\/p>\n\n\n\n

Many medical school graduates of my generation recited the oath that\u00a0Dr. Louis Lasagna,<\/strong>\u00a0Dean of Tufts University Medical School,\u00a0composed<\/a><\/strong>\u00a0in 1964. The oath\u00a0pays proper fealty to patients\u2019 privacy and to treat the whole patient<\/strong>\u2014not just a\u00a0set of lab tests or x\u2011rays. And it pledges to \u201cnot play at God.\u201d But the oath makes\u00a0no references to patients\u2019 freedom and autonomy.<\/strong><\/em><\/p>\n\n\n\n

Since the 1990s, many medical schools have added \u201cwhite coat ceremonies\u201d to the list of medical school rituals. These are ceremonies for incoming classes of medical students, where they also recite a version of the Hippocratic oath.\u00a0 The 2019 white coat ceremony oath for\u00a0Harvard Medical School and Harvard School of Dental Medicine<\/a><\/strong>\u00a0vowed to \u201cplace ethics and equity at the core of each patient interaction,\u201d\u00a0\u201ccombat structural oppression,\u201d \u201cpromote social justice,\u201d<\/strong>\u00a0and \u201cleverage our position of privilege to\u00a0confront health inequities.\u201d<\/strong>\u00a0No mention of patients as autonomous individuals.<\/strong><\/em><\/p>\n\n\n\n

Among the most controversial oaths was the white coat ceremony oath taken last September by incoming medical students at the\u00a0University of Minnesota Twin Cities Medical School<\/a>.<\/strong>\u00a0After\u00a0noting that the Medical School \u201cis located on Dakota land\u201d<\/strong>\u00a0and committing to \u201cuprooting the legacy and perpetuation of structural violence within the healthcare system,\u201d the\u00a0students pledged to \u201chonor all indigenous ways of healing<\/strong>\u00a0that have been historically marginalized by Western medicine\u201d\u00a0and<\/strong>\u00a0did not only commit to healing the sick but\u00a0to \u201chealing our planet and communities.\u201d<\/strong><\/em><\/p>\n\n\n\n

None of the oaths, dating back to the original, make more than a passing mention of respect for patients as autonomous, sovereign adults. All of them smack of paternalism. None of these oaths<\/strong> prioritize or consistently apply a commitment to individual patient autonomy, including respect for patients\u2019 rights<\/strong> to self\u2010\u200bmedicate and to seek treatment from any health care provider they choose\u2014an oath that states, for example,<\/strong><\/em><\/p>\n\n\n\n

\u201cEven if they act against my advice and I disapprove of their choices, I will respect the right of my patients as autonomous adults to self\u2010\u200bmedicate and oppose any laws and regulations that force them to seek my permission\u2014or permission from any other health professional, through a prescription or otherwise\u2014to consume medications or treatments according to their independent judgment.\u201d<\/em><\/p>\n\n\n\n

Today\u2019s medical students\u00a0should reject being forced to take oaths<\/a><\/strong>\u00a0that have nothing to do with patient care. Instead, a\u00a0noble profession should require its students and graduates to swear an oath revering patients\u2019 rights and autonomy.<\/em><\/p>\n\n\n\n
